This hospital allows international travellers to take Covishield both the doses in a gap of 28 days. 1. For first dose- a) need to visit DC office, fill up the annexure 3 form, show the travel proof (visa/employment letter etc) write a letter (on a white sheet) with all the details, get signatures from DC and the seal B) carry all the signed docs and proceed to DHO (office) beside Harsha showroom, BH road, get signed there from health officer C) go to Tunga nagar govt hospital, show the documents, (carry the passport original and aadhar card original), and mobile phone (as you need to provide OTP which you would receive), and get ur first dose done 2) for second dose in 28 days: instead of annexure 3, it will be annexure 4 and annexure 5 along with the same procedure mentioned above.
