Very hard to find this location. Today i got vaccinated there, if you have booked slot online, just go there and get vaccinated. Too rush, you have to stand in long queue for your number. Opposite to "OYO SAPNA HOTEL" there is a gali, under 100 meter there is the exact location. It's just dispencery or UPHC.
