It's a Public Health Centre at Transport Nagar. I went there for COVID vaccination for kids but despite vaccination appointment from 9 to 10 am. It opens after 10 am and by 10:30 they receive daily dose of vaccine. Usually, post this only they are able to start. This is why it is immaterial, if you have got an appointment done online or not, they'll do the registration afresh.
